QR-code makers put the minimum on a card at 20 to 25 mm; this tool fails a code under 20 mm and flags one under 25 mm. The 4-module quiet zone required by ISO/IEC 18004 is drawn inside this measurement.

One ink on plain stock is usually the simplest run to print. If the design still works there, it should hold up on richer stock too. Foil raises the organisation to 10 pt, the minimum MOO gives for foil and spot gloss.
